Should you be dissatisfied with any aspect of your care please contact the practice manager. If you are not happy with the response you receive please raise your concerns in writing with: NHS England, PO Box 16738, Redditch, B97 9PT or email england.contactus@nhs.net.

If you still feel your concerns have not been fully resolved you may raise a complaint with the Health Service Ombudsman by calling 0345 015 4033.
